At the time of your cosmetic consultation there is a modest fee of $100. If you schedule a procedure over $1,000 this fee will be applied to your total charges. Our office accepts cash, checks, Visa and MasterCard. We request if you are considering facial rejuvenation you should bring in several photographs of yourself when you were younger. If you have had surgery and are considering revisional or corrective surgery please bring a copy of your operative report(s) and your pre-operative photographs.

You will be examined by Dr. Culbertson not just a staff member, photographs will be taken and Dr. Culbertson will go over his action plan for your issues / concerns. Our actual patients with pre and post operative surgical results are presented for your review. Feel free to bring a list of questions and concerns for Dr. Culbertson and our staff to answer at the time of your visit. A tour of the Iris Surgical Center is made available for cosmetic patients so, you will be familiar with the setting of your surgical procedure.

To schedule a surgical procedure cosmetic patients are asked to place a scheduling fee. All fees for the package price for cosmetic surgical procedures are requested to be paid in full 2 weeks prior to the date of surgery. If you are a reconstructive surgical patient or your surgery is covered through an insurance carrier then, your financial responsibilities will be determined by your policy. We will be glad to answer any questions you have about your financial responsibilities and our financial policies.
